Initial Occupational Therapy Assessment - £450

This includes:

• Up to 1.5 hours face to face time with you in your home
• A detailed Occupational Therapy Assessment 
• Discussion of your difficulties, needs and goals
• Personalised recommendations
• Equipment and/or adaptation recommendations where appropriate
• An agreed plan for next steps
• A personalised Occupational Therapy report following the assessement

Follow-up Occupational Therapy appointment - £90 per hour

A follow-up appointment may be approprioate if you need further support after your initial assessment.

This could include:

• Reviewing your progress or changing needs
• Assessing newly purchased or installed equipment
• Checking that equipment is suitable and being used safely
• Providing advice and demonstrations
• Reviewing transfers, mobility or other everyday activities
• Reviewing previously recommended adaptations
• Making further recommnedations where needed
• Joint assessments - with specialist equipment reps or housing providers for example

Additional Report or Administractive Work - £45 per hour

Additional professional work outside the initial assessment package may be charged at £45 per hour. This will be discussed with you beforehand where applicable.

Travel

Travel within 25 miles of my base is included in the appointment fee.

For appointments more than 25 miles away, an additional travel charge will apply. This will be discussed and agreed with you before your appointment is booked.
