I provide functional Occupational Therapy assessments and solutions to support safe environments and independent living in your own home or care setting.

Occupational Therapy assessments

I will take the time to understand the difficulties you are experiencing and how they are affecting your day to day life.  

An assessment can look at areas such as:

  • Mobility and moving around safely at home
  • Transfers, such as getting in and out of bed, bath, chairs or the toilet
  • Personal care, including washing, dressing and using the bathroom
  • Access around your home, including stairs, doorways (main and internal) and different rooms
  • Falls risks and safety within the home
  • Managing everyday activities and maintaining independence
  • Equipment needs, where the right equipment may make everyday tasks easier or safer
  • Home adaptations, where changes to your environment could support your independence

 

What you will recieve

Following your assessment, you will recieve a personalised Occupational Therapy report outlining:

  • The outcome and key findings from your assessment
  • Agreed actions
  • Personalised recommendations
  • An agreed plan for next steps

 

Equipment recommendations

I can assess your needs and recommend equipment to help you manage everyday activities more safely, comfortably and independently.

This may include support with:

  • Getting in and out of bed or chairs
  • Moving around your home safely
  • Washing, bathing and using the toilet
  • Dressing and undressing
  • Preparing and eating meals
  • Reaching, bending, or accessing items around your home
  • Managing fatigue or reduced strength
  • Reducing the risk of falls
  • Making everyday tasks easier following an illness, injury or change in ability

Equipment may include: bathing and toileting equipment, mobility and transfer aids, seating, bed equipment, kitchen aids and other specialist equipment depending on your individual needs.

Minor Adaptations

Small changes to your home can make a big difference to your safety, independence and ability to manage everyday activities.

I can assess your needs and recommend practical adaptations to help you use your home more safely and comfortably.

This may include:

  • Grab rails and additional handrails
  • Additional support around steps and entrances
  • Small changes to help with transfers, mobility and personal care
  • Other simple adaptations based on your individual needs

I will consider how you use your home, the difficulties you are experiencing and what changes could make everyday activities easier and safer.

Major Adaptations

Sometimes more significant changes to your home are needed to help you remain safe, independent and able to manage everyday tasks.

I can assess your needs and provide professional Occupational Therapy recommendations for major adaptations to you home.

This may include:

  • Level access showers or accessible bathrooms/wet-rooms
  • Ramps and improved access to your home
  • Kitchen adaptations

I will consider your individual needs, how you use your home and the activities that are important to you when making recommendations.  Where appropriate, I can make recommendations to support discussions with contractors, housing providers or other professionals involved in the adaptation.

Moving & Handling Assessment

I can assess how you move around your home and identify ways to make transfers and everyday movement safer, more comfortable and more manageable.

This may include assessing difficulties with:

  • Changes in strength, balance or mobility
  • The use of mobility equipment or specialist equipment
  • Gettting from bed to chair
  • Support family or carer may need to provide

Where appropriate, I can recommend equipment, adaptations or alternative ways of completing transfers and moving around safely.

My recommendations are tailored to your individual abilities, your home environment and the support available to you.

Falls Prevention & home safety

If you have experienced a fall, feel unsteady on your feet or are worried about falling, I can assess the factors that may be affecting your safety at home.

I can look at how you move around your home and identify potential risks or difficulties that could increase your risk of falling.

This may include looking at:

  • Moving safely around your home
  • Getting in and out of chairs, bed, the bath or shower
  • Managing stairs or steps
  • Getting on and off the toilet
  • Lighting, flooring and other environmental factors
  • Position of furniture and everyday items
  • Appropriate equipment or mobility aids
  • Balance, mobility and confidence when moving around

Following the assessment, I can provide practical recommendations tailored to you and your home.  This may include advice about equipment, minor or major adaptations, safer ways of completing everyday activities and strategies to reduce potential falls risks.
